Tamoxifen is an antagonist of the estrogen receptor in breast tissue via its active metabolite, 4-hydroxytamoxifen. Platinib is a kinase inhibitor of wild-type RET and oncogenic RET fusions ( CCDC6-RET ) and mutations ( RET V804L , RET V804M and RET M918T ) with half-maximal inhibitory concentrations ( IC50s ) of less than 0.5nm . In the purified enzyme analysis, platinib inhibited DDR1 , TRKC , FLT3 , JAK1-2 , TRKA , VEGFR2 , PDGFRb and FGFR1 at higher concentrations, but it could still meet clinical requirements at Cmax . In cell assays, Platinib inhibited RET at approximately 14- fold, 40- fold, and 12- fold lower concentrations than VEGFR2 , FGFR2 , and JAK2 , respectively .

Certain RET fusion proteins and activating point mutations can drive tumorigenic potential through overactivation of downstream signaling pathways, resulting in uncontrolled cell proliferation. Platinib has antitumor activity in cultured cells and animal tumor implant models containing cancerous RET fusions or mutations, including KIF5B-RET , CCDC6-RET , RET M918T , RET C634W , RET V804E , RET V804L , and RET V804M , platinib can prolong the survival time of mouse models with intracranial implantation of tumors expressing KIF5B-RET or CCDC6-RET . 

【Indications】

(1 ) For the treatment of adult patients with advanced RET fusion-positive non-small cell lung cancer;

(2 ) For the treatment of adults and pediatric patients 12 years of age and older with advanced or metastatic rearranged transfection ( RET ) mutant medullary thyroid carcinoma ( MTC ) who require systemic therapy and are refractory to radioactive iodine ( If radioactive iodine is applicable), adult and pediatric patients 12 years of age and older with advanced or metastatic RET fusion-positive thyroid cancer ( TC ) .
